Eligibility for Services

Eligibility for services varies depending on each individual’s needs, and health plan/grant coverage. Grant funding may be available to provide services at little to no cost — contact our Enrollment Department for additional information on eligibility.”

Health Plans

We accept AHCCCS health plans and Medicare, in addition to Private/commercial insurance health plans (See attached list of private insurance companies and publicly funded plans available to members).

COPE also offers a Sliding Fee Scale to all income-eligible, uninsured, or under-insured patients. No one will be denied access to services due to inability to pay. There is a discounted/sliding fee schedule available based on family size and income. For additional information call 520.792.3293.

What to Expect at the Intake Appointment

During your first appointment with COPE, an initial assessment will be completed to determine your individual healthcare needs. 

What to Bring

The following items are required for scheduled intake appointments:

State issued ID

(driver’s license, passport, etc.)

Insurance card

Current list of medications

Emergency and primary care physician contact information
